This Privacy Policy explains how TFT Legal Service (“we,” “us,” or “our”) collects, uses, and shares personal information through depohelp.org (the “Website”), operated under the name Depo-Provera Help. It applies to information collected before you are represented by any law firm.

1. Who we are

TFT Legal Service operates Depo-Provera Help, a legal marketing and attorney referral service. We are not a law firm, and we do not provide legal or medical advice. When you ask for a case review, we forward your information to one or more participating attorneys or law firms so they can evaluate your request and contact you. If you sign an engagement agreement with a law firm, that firm’s own privacy practices and the attorney-client relationship govern the information you share with it from then on.

2. Information we collect

• Contact details and identifiers you enter, such as your name, phone number, email address, mailing address, state, ZIP code, and date of birth.
• Information about your situation that you choose to share, such as whether you received Depo-Provera or a generic depot medroxyprogesterone acetate injection, approximately when and for how long, whether you were diagnosed with a meningioma or another condition, the treatment you received, and whether you already have a lawyer. This is health information, and we treat it as sensitive. We use it only to evaluate and respond to your request.
• Consent records, such as the consent language you agreed to and the time you submitted it.
• Device and usage information collected automatically, such as IP address, approximate location derived from your IP address, browser and device type, pages viewed, the referring page, and how you interact with the case review questionnaire (including answers you select before you finish).

We do not ask for Social Security numbers, full medical records, or government ID on this Website. Please do not send them to us.

3. How we collect it

• From you, when you use the case review assistant (questionnaire) on our homepage, or when you email or call us.
• Automatically, through cookies, scripts and similar technologies used by the Website and by the questionnaire provider.
• From service providers that help us verify consent, prevent fraud and spam, and confirm the accuracy of submissions.

4. How we use information

• To respond to your request and connect you with a participating attorney or law firm for a free case review.
• To contact you by phone, text message, or email about your request, as you authorize.
• To keep records of your consent and to meet legal and regulatory obligations.
• To detect and prevent spam, fraud, and abuse, and to keep the Website secure.
• To understand how the Website performs and to improve it, using aggregated or de-identified information where possible.

5. How we share information

We share personal information only as described here:

• Participating attorneys and law firms (and their intake teams), so they can evaluate your request and contact you. You direct this disclosure when you submit the questionnaire.
• Service providers that work on our behalf under contract, including the provider that hosts the case review questionnaire and stores submissions (LeadBot / GetLeadForms), and providers of consent verification, spam protection, website hosting, and email delivery.
• Legal and safety reasons: to comply with law, subpoenas or court orders, to protect rights, safety and property, or to investigate fraud.
• Business transfers: as part of a merger, acquisition, or sale of assets, subject to this policy.

We do not sell your personal information for money, and we do not share your health information with advertisers.

6. Cookies and similar technologies

The Website itself does not set advertising cookies. The case review questionnaire, provided by a third party, may use cookies, local storage, and scripts to run the questionnaire, remember your progress, prevent fraud, record consent, and measure performance. You can block or delete cookies in your browser settings; if you do, the questionnaire may not work properly.

7. Calls, texts and emails

If you submit your contact details and agree to the consent language shown in the questionnaire, you may receive calls, text messages, and emails about your request, which may use automated technology. Consent is not a condition of any purchase or service. Message and data rates may apply. You can reply STOP to any text to opt out, or email [email protected].

8. How long we keep information

We keep personal information only as long as reasonably necessary for the purposes above: generally while your request is being evaluated and then for the period needed to keep consent and compliance records and resolve disputes (typically up to five years), unless a longer period is required by law. We then delete, de-identify, or securely archive it.

9. Security

We use reasonable administrative, technical, and physical safeguards, including encrypted (HTTPS) connections. No method of transmission or storage is completely secure, so we cannot promise absolute security.

10. Your choices and rights

You may ask us to: confirm whether we process your personal information and give you a copy; correct inaccurate information; delete your information; and stop contacting you. We will verify your request using information you previously gave us and respond within the time required by law. You will not be treated differently for exercising your rights. If we deny your request, you may appeal by replying to our decision.

11. State privacy rights

Residents of certain states (for example California, Colorado, Connecticut, Oregon, Texas, Virginia, and others) have specific rights under state privacy laws, which may include the rights to know, access, correct, and delete personal information, to opt out of the sale or sharing of personal information or targeted advertising, and to limit the use of sensitive personal information (such as health information). Some states, such as Washington and Nevada, have additional rules for consumer health data. We use health information only to respond to your request and share it only as described in Section 5. Third-party code used by the questionnaire may receive device and usage information, which some state laws treat as “sharing” or “targeted advertising”; to opt out, email [email protected] with the subject “Do Not Sell or Share,” or turn on a Global Privacy Control (GPC) signal in your browser. You or an authorized agent may submit a request by email or phone as described in Section 10.

12. Children

This Website is intended for adults. We do not knowingly collect personal information from children under 13 (or under 16 where state law requires) without the consent required by law. If you believe a child has given us information, contact us and we will delete it.

13. Changes to this policy

We may update this policy from time to time. We will post the updated version on this page and change the “Last updated” date. If changes are material, we will provide a more prominent notice.
